Most people experience some form of anorectal disorder (fissure & fistula) during their lifetime.

What is Anorectal ?

The term “Ano” refers to the anus, which is the opening at the end of large intestine through which stool passes. And “Rectal” refers to the rectum, which is the area of large intestine above the anus. Ano-rectal, is the disorder that occurs at the junction of the canal (anal canal) and the rectum.

The anus is the opening where the gastrointestinal tract ends and exits the body. The anus starts at the bottom of the rectum, the last portion of the colon (large intestine). The anorectal line separates the anus from the rectum. Tough tissue called fascia surrounds the anus and attaches it to nearby structures. Circular muscles called the external sphincter ani form the wall of the anus and hold it closed. Glands release fluid into the anus to keep its surface moist. A plate-like band of muscles, called the levator ani muscles, surround the anus and form the floor of the pelvis. A network of veins lines the skin of the anus.

What is Anorectal Malformation ?

Anorectal anomalies are the problems affecting the structures of the anus and rectum. A person with an anorectal problems have some deformative features of the anus or rectum, known as anorectal malformation.

Anal stenosis means narrowing of the anal opening that makes it difficult for stool to pass through easily. The stool may exit under. Treatment of this disorder involves dilation of the anal opening.

  • Imperforate Anus:

Imperforate anus, also known as, Anal atresia is a birth defect caused by a malformed rectum, absence or abnormal localization of the anus. The rectum or the colon may be connected to the vagina or the bladder by a tunnel (fistula).

  • Anal Fistula:

  • Anal Cancer:

Anal cancer occurs when skin cells grow out of control in the anus, the area just a few inches inside the butt. Bleeding is often the first sign of the disease.

Symptoms of Fissure & Fistula

Causes of Fissure & Fistula

It is the important part of treating anorectal (fissure-fistula) disorders is to determine the cause of the condition.

Constipation is the main key cause of major anorectal disorders. Passing hard or large stools results inflammation of the anorectal area. Constipation during bowel movements, Chronic diarrhea, Childbirth is another cause for anorectal disorder (fissure-fistula). The other causes are :

  • Blocked glands in the anal area.
  • Infection of an anal fissure.
  • Sexually transmitted infection.

Treatments of Fissure & Fistula in Ahmedabad, Gujarat

The most common advices given to the patients with less severe disorders include a high-fiber diet, increased water intake and an application of ointment while pricking/itching sensation. Both patients and physicians should be aware of the need for adequate assessment of all anorectal symptoms because of the high incidence of chronic stages.

The treatment includes warm sits baths, drinking more water, increasing fiber in the diet, topical creams, and taking stool softeners. Sometimes exercise is recommended to soften stool.

Surgical Treatments for Fissure

  • Chemical Sphincterotomy: Not strictly speaking surgery, this is a new treatment that involves using BOTOX® injections to relax the anal sphincter muscle; it seems promising but its effectiveness compared to traditional surgery is still under review.
  • Lateral Internal Sphincterotomy: This is most commonly performed operation for anal fissures. This is a relatively minor procedure but is still done under general anesthetic. The surgeon makes a small cut into the internal anal sphincter, the band of muscle that usually holds the anus closed. This releases pressure on the tear, so that it can heal over the next couple of months. This operation is performed as a last resort because surgeons are very worried about weakening the sphincter and causing mild incontinence. However, it is effective at healing anal fissures in over 90% of cases.
  • Anal Advancement Flap: This can be performed when all other options have been unsuccessful.

Surgical Treatments for Fistula

  • Fistulotomy: In a fistulotomy the surgeon first probes to find the fistula‘s internal opening. Then the tract is cut open and is scraped and its contents are flushed out, then its sides are stitch to the sides of the incision in order to lay open the fistula.

A more complicated fistula, such as a horseshoe fistula (where the tract extends around both sides of the body and has external openings on both sides of the anus), is treated by usually laying open just the segment where the tracts join and the remainder of the tracts are removed.

The surgery may be performed in more than one stage if a large amount of muscle must be cut. The surgery may need to be repeated if the entire tract can’t be found.
  • Advancement Rectal Flap: A surgeon may core out the tract and then cut a flap into the rectal wall to access and remove the fistula’s internal opening then stitches the flap back down. This is often done to reduce the amount of sphincter muscle to be cut.
  • Seton Placement: A seton (silk string or rubber band) is used to either:

    • Create scar tissue around part of the sphincter muscle before cutting it with a knife

    • Allow the seton to slowly cut all the way through the muscle over the course of several weeks

The seton may also aid in the drainage of the fistula.

  • Fibrin Glue or Collagen Plug: In some cases, fibrin glue, made from plasma protein, may be used to seal up and heal a fistula as opposed to cutting it open. The glue is injected through the external opening after clearing the tract and stitching the internal opening closed. A plug of collagen protein may also be used to seal and close the fistula tract.
